Enrollment Requirements & Guidelines

Higher Education · Incoming Freshmen

Documentary Requirements

  • Original Grade 12 Report Card (Form 138) with Learner Reference Number (LRN)
  • PSA-authenticated Birth Certificate (original)
  • Certificate of Good Moral Character (original, issued by previous school)
  • Medical / Health Certificate from a licensed physician
  • 2 pcs. 2×2 ID photo (white background, taken within 3 months)
  • Photocopy of parent’s or guardian’s valid government-issued ID
  • Accomplished TLC Admission Application Form
Note: New enrollees are required to take and pass the TLC College Admission Test (CAT) before final enrollment. Exam schedules are announced at the start of each enrollment period.

Enrollment Process

  1. 1Obtain and accomplish the TLC Admission Application Form from the Admissions Office or download it from the website.
  2. 2Submit all required documents to the Admissions Office for initial screening and verification.
  3. 3Take the TLC College Admission Test (CAT) on your scheduled date.
  4. 4Await the release of admission results. Qualified applicants will be notified by the Admissions Office.
  5. 5Proceed to the Registrar’s Office for course and subject enrollment upon admission confirmation.
  6. 6Pay the required fees at the Cashier’s Office and keep your official receipt.
  7. 7Claim your enrollment form / Certificate of Registration (COR).
Higher Education · Transferees

Documentary Requirements

  • Certificate of Transfer Credential / Honorable Dismissal (original)
  • Official Transcript of Records (OTR) from previous institution
  • Certified true copy of grades for all completed semesters
  • Certificate of Good Moral Character (original, from previous school)
  • PSA-authenticated Birth Certificate (original)
  • Medical / Health Certificate from a licensed physician
  • 2 pcs. 2×2 ID photo (white background, taken within 3 months)
  • Photocopy of parent’s or guardian’s valid government-issued ID
  • Accomplished TLC Admission Application Form
Note: Transferee slots are subject to availability per program. The Registrar’s Office will assess the transferability of units earned from the previous institution before final enrollment is confirmed.

Enrollment Process

  1. 1Request your Honorable Dismissal and OTR from your previous school before visiting TLC.
  2. 2Submit all required documents to the Admissions Office for evaluation.
  3. 3Undergo a subject credit evaluation with the Registrar’s Office to determine your year level and applicable credited units.
  4. 4Take the TLC College Admission Test (CAT) if required by the Admissions Office.
  5. 5Receive your enrollment clearance and proceed to course and subject enrollment at the Registrar’s Office.
  6. 6Settle all fees at the Cashier’s Office and secure your official receipt.
  7. 7Claim your Certificate of Registration (COR).
Higher Education · Continuing Students

Documentary Requirements

  • Accomplished Enrollment Form (available at the Registrar’s Office)
  • Previous semester’s grades or Evaluation Sheet
  • Financial clearance from the Cashier’s Office (no outstanding balance)
  • Library clearance (no unreturned books or overdue fines)
  • Updated 2×2 ID photo (if current school ID has expired)
  • Duly accomplished department / program clearance slip
Note: Students with outstanding academic or financial obligations from the previous semester must settle these before they may proceed with re-enrollment. Consult your Program Head for academic advising prior to enrollment.

Enrollment Process

  1. 1Secure your grades and evaluation sheet from the Registrar’s Office or your Program Head.
  2. 2Obtain an Enrollment Form and have it signed by your Program Head or Academic Adviser.
  3. 3Settle any outstanding balance at the Cashier’s Office and obtain financial clearance.
  4. 4Submit your accomplished Enrollment Form to the Registrar’s Office for subject encoding.
  5. 5Pay the applicable tuition and miscellaneous fees at the Cashier’s Office.
  6. 6Claim your Certificate of Registration (COR) for the new semester.
Higher Education · Returning Students

Documentary Requirements

  • Letter of Intent to Return (addressed to the College Registrar)
  • Certificate of Good Moral Character (newly issued)
  • Accomplished Enrollment / Re-Admission Application Form
  • Copy of last enrolled semester’s grades from TLC
  • Medical Certificate from a licensed physician (required if Leave of Absence was health-related)
  • 2 pcs. 2×2 ID photo (white background, taken within 3 months)
  • Financial clearance from the Cashier’s Office (if applicable)
Note: Returnees may be required to undergo an interview with the Dean or Program Head before re-admission is approved. Subjects and credited units are subject to re-evaluation based on the current curriculum.

Enrollment Process

  1. 1Write a Letter of Intent to Return and submit it to the Registrar’s Office with your supporting documents.
  2. 2Await evaluation of your re-admission application by the Registrar and Program Head.
  3. 3Attend an interview with the Dean or Program Head if required.
  4. 4Upon approval, secure your Enrollment Form and have it signed by your Program Head.
  5. 5Settle any remaining financial obligations from your previous enrollment at the Cashier’s Office.
  6. 6Complete subject enrollment with your Program Head and pay all applicable fees.
  7. 7Claim your Certificate of Registration (COR).
Basic Education · K–12

Kindergarten & Grade I

  • PSA-authenticated Birth Certificate (original)
  • Report Card from previous school (for Grade 1 entrants)
  • Certificate of Good Moral Character (for Grade 1)
  • 2 pcs. 2×2 ID photo (white background)
  • Photocopy of parent’s or guardian’s valid ID

Grade 7 (Junior High School)

  • Original Grade 6 Report Card (Form 138) with LRN
  • PSA-authenticated Birth Certificate (original)
  • Certificate of Good Moral Character
  • 2 pcs. 2×2 ID photo (white background)
  • Photocopy of parent’s or guardian’s valid ID

Grade 11 (Senior High School)

  • Original Grade 10 Report Card or Certificate of Completion
  • PSA-authenticated Birth Certificate (original)
  • Certificate of Good Moral Character
  • SHS Voucher / ESC Certificate (for public school completers)
  • 2 pcs. 2×2 ID photo (white background)
  • Photocopy of parent’s or guardian’s valid ID

Enrollment Process (All BED Levels)

  1. 1Visit the Basic Education Department office and obtain an Enrollment Form.
  2. 2Submit all required documents to the BED admissions personnel for verification.
  3. 3Await confirmation of your enrollment slot (slots are limited per grade level).
  4. 4Proceed to the Cashier’s Office to pay tuition and miscellaneous fees.
  5. 5Present your official receipt to the BED office to finalize enrollment.
  6. 6Claim your class schedule and other enrollment documents.

For Continuing BED Students

  • Previous school year’s Report Card (Form 138)
  • Financial clearance from the Cashier’s Office
  • Accomplished Enrollment Form (from BED Office)
  • Updated 2×2 ID photo if school ID has expired
SHS Voucher Program: Grade 10 completers from public schools may qualify for the DepEd SHS Voucher (GASTPE), which provides a tuition deduction. Bring your voucher certificate during enrollment to avail of this benefit.
General Reminders

Before you enroll

Document Preparation Tips
  • Prepare original copies of all documents. Photocopies may be accepted as support but originals will be verified.
  • Ensure your PSA Birth Certificate is authenticated (with a green security paper). An NSO-issued copy is acceptable.
  • ID photos must be taken within the last 3 months, in white background, and in school or formal attire.
  • Check that all documents bear the official school seal and signature of the authorized signatory from your previous school.
  • Bring extra photocopies of each document in case additional copies are needed.
Important Policies
  • Enrollment is not final until all fees have been paid and the Certificate of Registration (COR) has been issued.
  • Students with incomplete documents may be given a grace period, subject to the approval of the Admissions Office.
  • Slots per program and grade level are limited. Enrollment is on a first-come, first-served basis.
  • The school reserves the right to deny enrollment to applicants who do not meet the admission requirements.
  • All enrolled students are bound by the TLC Student Handbook and must adhere to school policies and standards of conduct.
